ASTHMA CONTROL TEST FOR PEOPLE AGE 12+

Not sure how well you’re controlling your asthma? This test can help you pinpoint where your control is great and where you might need help. Complete each question, then click Submit for Results. Print a copy of the completed test and bring it with you to your next doctor’s appointment. Your asthma may change over time, so make regular testing a part of your routine.

This test is designed for people 12 years or older. Children age 4-11 should take the asthma control test for children. Only you will have access to your answers, which won’t be saved on this website. You can also print the test and complete your answers that way.

The answers to these seven questions will give your child’s doctor a good idea of his or her asthma control. Ask your child to answer the first four questions, then complete the remaining three questions on your own. When you’re finished, click All Done. You can print a copy of the test results and bring it with you to your child’s next doctor’s appointment.

This test is designed for children 4 to 11 years old. People age 12 or older should take the asthma control test for adults. Only you will have access to your answers, which won’t be saved on this website.
